As part of the litigation of the BP Texas City Refinery Benzene release from April to May of 2010, the Court has ordered each Plaintiff to fill out and sign multiple authorization sheets. These authorizations allow for the release of your medical, education, tax, social security, and employment records to the Defendant, British Petroleum. An authorization packet must be filled out for EACH AND EVERY MEMBER OF YOUR FAMILY that has signed up as a client with The Pinkerton Law Firm in connection with the April-May 2010 Benzene Release.

There are two sets of authorizations, click the above link for adults for anyone over the age of 18 and click the above link for minors for anyone under the age of 18 years. You will notice that we have inserted grey X by various lines on the authorizations. A grey X indicates the required information fields you need to fill out to the best of your ability. Also, EVEN IF AN AUTHORIZATION DOES NOT APPLY TO YOU, FILL IT OUT. If you don't have the required information, then leave it blank and simply sign the required signature line. DO NOT DATE THE AUTHORIZATIONS.

Please print out the authorizations and fill in the grey boxes we have inserted. Once you have completely filled out the Authorizations, please return them to the Pinkerton Law Firm, PLLC, by one of the following methods:

1. Scan and email the authorizations as an attachment to bprelease@chadpinkerton.com;
2. Fax the authorizations to our firm. Fax number is 281-332-1008;
3. Mail the authorizations to our office at 711 W. Bay Area Blvd., Ste 530, Webster, Texas 77598, or;
4. Deliver the authorizations in person between 8am-5pm, Monday through Friday, to our office at 711 W. Bay Area Blvd., Ste 530, Webster, Texas 77598.

Also, we will be setting up a satellite office in Texas City. This office will be located in the Holiday Inn Express Conference Room at 2440 Gulf Freeway Frontage Road, Texas City, Exit 15 and it will be open the next two weekends from 10:00 am to 4:00 pm January 28th & January 29th, and from 10:00 am to 4:00 pm February 4th & February 5th. You will be able to deliver your authorizations to this location, and should you have any questions, we will have staff at this location to answer your questions.
